Non-Resident Services Plan Scenario 4: Student has an active plan (IEP or SP) in Phoenix – previously received services as a resident but was never exited (Current Phoenix Status is "Active (Special Ed)")

This scenario is for a student who previously received services from SSD as a resident but has not been exited from Phoenix.

Case Manager

In Phoenix, change Case Manager to the correct Teacher-Level Case Manager (if it has not been changed).

Note: When the student’s record is located in Phoenix, an IEP may appear on Scheduled Events. DO NOT complete this event. The IEP will be deleted when the Non-Resident Services Plan event is created by the SEIMS Department.

Locate the Non-Resident Services Plan file on the Non-Resident Services Plan Information and Forms page of the Technology Services Knowledge Base.

Download the Non-Resident Services Plan and save it to your desktop (The plan is a fillable PDF.).

Complete the Non-Resident Services Plan.

Attach the completed Non-Resident Services Plan file to a Technology Service Desk ticket in the Phoenix section of the Service Desk Portal. Select Create event and drag the completed file to the ticket. The file can also be attached to an email using your SSD email account, and sent to technologyservices@ssdmo.org

SEIMS Department

Create an Exit event using the exit date reported by the partner district as the Scheduled and Due Dates (Student Data has access to this date). The date must be prior to the Meeting Date of the NRSP and allow time to complete the Transfer event. If the student was not exited by the partner district, the Scheduled Date must be prior to the Meeting Date of the NRSP and allow time to complete the Transfer event (date sequence must be Exit event Date, Transfer event Date, NRSP Meeting Date).

Complete Exit form selecting Exited Special Education > “Moved out of St. Louis County known to be continuing Special Education”. Enter student’s current resident district.

Complete Comments section indicating the student has moved outside of St. Louis County and will now receive services through a Non-Resident Services Plan.

Lock the Exit event.

SEIMS Department

After the student is exited create a Transfer event. The Scheduled Date should be after the exit but prior to the NRSP. Assign the event to the person completing it. In this scenario the Transfer Event will not be completed by the SPED Coordinator
(Note: the Transfer event can be created and completed on the same day as the Exit to prevent the student from being exited from ESI).

Open the Transfer event.

On Transfer Decision form select “In-state Transfer”.

Complete In-State Transfer form.

Enter the Scheduled Date of the Transfer event as the date IEP Requested and Received.

Enter the Scheduled Date of the Transfer event as the date Evaluation Report Requested and Received.

Enter the current Evaluation Date in Date of Evaluation Report.

Enter current IEP date in Date of IEP.

Select Option D (Both Evaluation Report and IEP Received).

Enter the Scheduled Date of the Transfer event as the date the evaluation report was Reviewed.

Select “Evaluation report accepted and IEP rejected.”

Enter the Name/role of individuals making decisions. Enter the name of the building SPED Coordinator and the person completing the form as SSD Representative. Enter a note in the Component District Representative field that says this event is being completed to meet system requirements.

Complete the Additional Information page explaining the scenario (student previously received services through an IEP as a St. Louis County resident but is now a non-resident and will receive services through a Non-Resident Services Plan).
Note: The Jira ticket number can be entered on the Additional Information page for further clarification.

Lock the Transfer event.

Notes

Notification of Meeting-Annual IEP (Transfer), Annual IEP (Transfer) and Reevaluation Decision events are created on Scheduled Events.

ESI Status: Active

SEIMS Department

Create the Non-Resident Services Plan event using the meeting date of the new NRSP as the Scheduled and Due Dates. Enter the Case Manager as the Primary Staff.

Open the Non-Resident Services Plan event.

On the Non-Resident Services Plan Decision form, select “Student receiving services through a Non-Resident Services Plan”.

Complete the date of the most recent Non-Resident Services Plan.

Save the form.

Attach the NRSP file to the Non-Resident Services Plan event.

Lock the Non-Resident Services Plan event.

Change the Primary Staff for the future Non-Resident Services Plan event to the Case Manager.

Delete the Notification of Meeting-Annual IEP (Transfer) and Annual IEP (Transfer) from Scheduled Events.

Notes

After locking the Non-Resident Services Plan event, a new Non-Resident Services Plan event is created on Scheduled Events with a due date one year, minus one/two days, from date of most recent Non-Resident Services Plan.

The Reevaluation Decision event appears on the Scheduled Events panel.
