Triennial Evaluation Documentation Form

Form information 

This form was previously referred to as the Reevaluation Waiver Form. The Triennial Evaluation Documentation Form documents the decision that a triennial reevaluation is not necessary. It is completed by the IEP Chair. 

The Parent/Guardian/Student (18+), Component District Representative, and Special School District Representative must all agree that a triennial evaluation is unnecessary. If they do not all agree, a reevaluation must be completed.

  1. The Date of most recent evaluation is pre-filled based on the date of the last evaluation

  2. Indicate if the student is a Voluntary Transfer Student (VTS) or a Non-Public (SNAP) student

  3. The Date of decision that a triennial evaluation is unnecessary is pre-filled with the Scheduled Date of the Reevaluation Decision Event. (The date cannot be edited on the form. If this is not the date the decision was made that a triennial evaluation is unnecessary, return to the Events screen, select the Edit icon for the Reevaluation Decision Event and change the Scheduled Date. When the form is reopened, the date will be updated. This date becomes the new Reevaluation Date. The next Reevaluation will be due 3 years from the date the decision was made that a triennial evaluation is unnecessary.)

  4. Enter the names based on role of who made the agreement

  5. Save

This form is used when the decision is that no reevaluation is necessary. Therefore, no Review of Existing Data form is created or completed.

If this form has been completed in error (if a Review of Existing Data is needed instead), it should be reset so it is not included in the final, locked event. Instructions for resetting forms are found on the Resetting Forms page.
