Prior Written Notice - Initial Services

Form information

The Prior Written Notice for Initial Services is in the Parent Consent-Initial Services event which is created with the Initial IEP when the Initial Eligibility event is created or when the Initial SP is created.  This Prior Written Notice is only used to obtain consent for initial services.

It is extremely important that the Initial IEP event is NOT locked before the signed Parent Consent-Initial Services Prior Written Notice is received from the parent. If an error appears in the locked Initial IEP, the error cannot be addressed in an IEP Correction event until after the Parent Consent-Initial Services event has been completed and locked.. This can be an issue if the parents request a change to the IEP before they will sign for consent. After receiving and documenting the parent's response to the Consent for Initial Services Prior Written Notice, the Initial IEP can be locked. The Initial IEP event must be locked before the Parent Consent-Initial Services event can be locked. Refer to the Initial IEP article for more information.

  1. Choose Personally Presented or Mailed.

  2. Enter the Date of the first notice in 1st Date sent or given.  If this is the second notice, do not delete the first date. Enter the Date of the second notice in 2nd Date sent or given.  Two attempts to obtain consent for placement are required.  If you choose to do a third attempt, enter that date in 3rd Date sent or given.

  3. The reason for the Notice is printed on the form – Proposed:  Initial Services and Initial Placement due to eligibility in the areas of

  4. Enter all areas in which the student qualifies for Special Education Services.

  5. In Description and explanation of the action enter both the initial services that will be provided and the initial placement category selected by the IEP team.

  6. Complete the other required fields.  The text boxes expand.

  7. Enter your Name, Title, and Contact Information.


  1. If the parent signs for consent, mark Consent.

  2. Enter the parent’s name as they signed it.

  3. Enter the Date the notice was signed.

  4. Enter the Date the notice was received.

  5. Save.

The dates entered in the 1st Date sent or given, Date Signed, and Date Received fields must be in the correct order. The Date Signed and Date Received cannot be future dates. If a date violates a rule, an error message will appear and the event cannot be locked until the error is corrected.

The original, signed copy of the Prior Written Notice is sent to Student Records at Special School District Central Office.


  1. If the PWN is returned indicating the parent refused consent for Initial Services, check Refused.

  2. Enter the parent’s name as it appears on the form. If the parent did not return the form but indicated in another way they are refusing Initial Services, type "Parent Refused" on parent signature line.

  3. Enter the date the parent signed the form or indicated they are refusing Initial Services.

  4. Enter the date the PWN was received or notice was received that the parent is refusing services.

  5. Save.


No Response / Final Attempt

If the parent does not respond to attempts to obtain consent, consult with your administrator before completing and locking this event.

  1. If, after multiple attempts to obtain consent, no response is received from the parent, check No Response and Final Attempt.

  2. Enter "No Response" in signature field.

  3. Enter the date the decision was made to no longer attempt to receive consent in both Date Signed and Date Received.

  4. Save.
