Placement Considerations - K12

This article assists users when completing the form.

Form information

  1. Choose Yes or No to indicate if the student will participate 100% of the time with non-disabled peers in the regular education environment.

  2. If the answer in step 1 is No, the next section becomes required.

    1. Enter the percent in both the regular education and special education fields. These numbers can be found on the Least Restrictive Environment section of the Federal 4 Summary screen.

      1. NOTE: Pay particular attention to the order of these fields. The field for "regular education" is first. This is different from the order in which the percents appear in the LRE section of the Federal 4 Summary Screen.

    2. In the next field describe why full participation is not appropriate at this time.  If there is not enough space, enter See Additional Information and put the reason(s) on the Additional Information Form.

  3. Choose the appropriate statement regarding Participation in Regular Physical Education.  If the student is not participating in physical education, select the reason or enter a reason in Other.

  4. Complete the Annual Consideration of Placement section.

    1. Mark all placement options Considered. Be sure to select Considered for Inside regular class at least 80% of time. This option must be marked for all K-12 students.

    2. In the Selected column, mark the appropriate placement based on the percent of time spent in the regular class or the student’s placement in a public or private separate school, a public or private facility, or home/hospital (homebound).

  5. Choose Yes or No to indicate if the student’s placement is as close as possible to the child’s home.

  6. If the answer in step 7 is No, explain why another setting is required.

  7. Save.

There are three Placement Considerations forms available.  The first (K12-1) is required.  The second and third (K12-2 and K12-3) are optional and used if the placement changes during the IEP.
