Form information 

The Record of Participation is used to document the names and roles of the individuals attending the eligibility determination meeting.

  1. Enter the Names of those who participated based on role.

  2. If not present at the meeting, check Not Present.

  3. Save

Specific Learning Disability
When the student meets eligibility criteria for Specific Learning Disability and it is selected on the Disabilities Screen, the response to the statement “The team has determined that this student meets criteria for a Specific Learning Disability“ auto-fills “Yes”. When the answer to this statement is Yes, the yes/no eligibility agreement boxes become active for all listed participants and can be completed based on eligibility agreement during the meeting by the individual participants.
