Transportation Decision Form

This article assists users when completing the form.

Form information

The Transportation Decision form is created when “The student requires transportation as a necessary related service” is selected on the Services Summary form. It is used to indicate the provider of the student's transportation. The Transportation form is created based on the selection made on the Decision form. 

  1. Choose the appropriate transportation provider.

  2. If Special School District or Partner District is chosen, the Transportation form appears on the Forms List panel. 

  3. If ParentPublic TransportationVICC, or Not Needed is chosen, the Transportation form is not created.

  4. Save.

If this form has been created, completed, and saved but is no longer needed, it can be reset so as not to be included in the final, locked IEP. Ensure that if any radial button is marked, you click the correct one for the student’s situation and save. Then, follow the instructions for resetting forms that are found on the Resetting Forms page.
