Summary of Performance Form

This article assists users when completing the form.

Form information

The Summary of Performance form is included in the Summary of Performance event, which is created by the user when needed. 

  1. Complete all required fields.

  2. Enter the Date the form is being completed.

  3. Some student information is auto-filled. Complete any fields that are not filled and correct any incorrect information in the editable fields.

  4. Enter the Date of Exit/Graduation.

  5. Indicate how the student graduated (Credits or Alternate Standards).

  6. Enter the Student’s Primary Disability.

  7. Enter the Case Manager name.

  8. The text fields in the Background Information, Academic Achievement, and Functional Performance sections expand.

  9. Make one Post-Secondary Goal choice in both the Education/Training and Employment sections.  Some choices will require a text box to enter further information.  These text boxes do not expand.

  10. Enter Agency Linkage information for both the Education/Training and Employment sections. The text fields expand.

  11. Multiple choices can be made in the Independent Living section.  If this section does not apply to this student, choose ‘Not Applicable’.

  12. If the Independent Living section was completed, enter Agency Linkage information. The text fields expand.

  13. Save.

  14. If a Prior Written Notice for Change of Placement due to graduation was not included in the student’s final IEP, complete the PWN included in the Summary of Performance event.

  15. Return to Event Overview.

  16. Click the Validation Check button. Address any errors that appear.

  17. Click Print to preview the form and proofread it.

  18. When the forms are completed correctly, Lock the event.

  19. Print a copy for the student.

The Summary of Performance (SOP) must be completed not sooner than 30 calendar days before and not later than 10 calendar days after the date the student will be exiting from school due to graduation or aging out at age 21. The event must be locked and printed before Student Data can exit the student.
