Student Profile

The Student Profile can be accessed anywhere in the application by selecting a student’s name when it is a link. The profile has 7 tabs:  Summary, Student Details, Program Summary, Events, Logs, Student Audits, and Watch List. Role permissions are required to access tabs and perform functions.

Summary

Summary screen has 3 panels:  Student Alerts, Program Snapshot, and Hot Tasks.

  • Student Alerts has charts that replicate the charts in Program Compliance on the dashboard except these represent information for the student. 

    • Staff Unassigned shows if the student has an unassigned Case Manager, Service Provider or Goal Responsible Staff

    • Meetings show any meetings that are past due, due in 7 or 8-30 days

    • Progress Reports show any progress reports that are past due, due in 7 or 8-30 days

  • Program Snapshot shows a brief summary of the student’s special education programming such as plan information, case management, disability and IEP information.

  • Hot Tasks show any events or progress reports that are past due or due in 7 days.  Details about an event can be viewed by selecting the eyeball.  The link for an event opens it.  In Progress Reports, multiple reports can selected by choosing a date or select each progress report to complete them.

Student Details

Student Details show student demographic and enrollment information.

Program Summary

Program Summary has menu options: Special Ed Summary, Plan History, Disabilities History and Case Manager History.

  • Special Ed contains details from the active plan, disability information and progress reports.

    • ‘Fly To’ (airplane) is used for easier access to specific sections 

    • Progress reports appear in Pending 10 days prior to the due date and remain until marked complete. They can also be manually added to this section.

      • To complete all progress reports, select the checkbox next to the Progress Due Date. To complete certain progress reports, select the checkbox next to an individual progress report.  Once progress reports have been selected, click ‘Go to Progress Reports’ to open the Progress Report Details window to complete them.

      • Progress Report Details show only progress reports assigned to the logged in user.  To view all progress reports you selected, uncheck ‘Assigned to Me’. 

        • Last Report is used to view content of the last progress report for a goal

        • Last Progress provides a reminder of the last progress score entered for this goal

        • Summary is used to enter the progress report summary by selecting ‘Enter Text’ to open the text field

        • Complete, when selected moves the progress report from Pending to Complete on the Program Summary screen

      • Progress Reports can be selected to be printed from either section - Pending or Complete.

  • Plan History shows details of all historical plans.  Select the row to view the details of a plan and the plan expands with associated goals, services, LRE, assessments, classroom accommodations and progress reports. Progress Reports are printed here from inactive plans.

  • Disabilities History shows a record of all disabilities and their statuses.

  • Case Manager History shows a history of all Case Managers with assignment dates. The current Case Manager has no end date.

Events

Events are a collection of related forms that address a specific educational need for a student such as an IEP.  Events can be opened to be viewed, completed and locked on this screen.

  • Scheduled Events are a list of recurring events that are generated by Phoenix based on set rules or added by a user.  “+New Meeting' can be used to add a new event. All events remain in Scheduled Events to edit until they are locked.

  • Locked Events are a list of events that have been completed and locked.  Plan Status, key event dates and staff member that completed the event are included. Locked Events can be viewed and printed. 

    • Event Type is the name of the event. If an attachment has been added to the event, a paperclip appears next to event name with the number of attachments when hovering over the paperclip.

      • Plan Status indicates the current status of the event. Current plans are ‘active’.  If an ‘active’ plan has expired, status will be highlighted in red

      • Completed Date is the date event was locked

      • Completed By is the primary staff who completed the event

      • Actual Date of an event is the scheduled date of event

      • Due Date is the date event was due

      • Each locked event has a menu:

        • View is used to view or print event forms and attachments. 

        • Show Admin Overrides is used to the reason for an admin override. 

        • Attachments is selected to view attachments added in this event.

View Latest will only be available if an Admin Override has been saved.  This differentiates between original event forms and modified event forms, also the menu is outlined in black to denote the change.  Select this option to view or print the most recent event forms.

Logs

Logs tab is where users can view attachments such as evaluations and IEPs added for transfer students.  Documents specific to a certain event such as assessment results are not added on this screen and should be uploaded as an Event attachment in Event Overview of the appropriate event.

Student Audits

Student Audits allows users to audit user activity in Phoenix that involves the student.  A log is shown based on a specified date range and data can be filtered by a word or phrase.

Watch List

The Watch List lists all staff members who have the student on their Watch List.  Students can be added or removed from a Watch List on this screen. All Watch Lists are cleared at the end of July.
