The Transfer Decision Form is completed by the Intake Department based on information provided by the partner district.  It is used to document whether the transfer is an in-state or out-of-state transfer.  Based on the selection, the appropriate form will be required. 

Form information

  1. Make the appropriate selection (In-state Transfer or Out-of-state Transfer)

  2. Save

Once this form has been saved the appropriate In-state or Out-of-state Transfer Form will appear as a required form within this event.
