In-State Transfer Form

This article will help users with the form.

Form information 

The In-State Transfer Student Documentation Form is used when a student transfers to a partner district from a non-partner school district in Missouri.

The Student Intake Department creates the event and completes the enrollment, demographic information and the records sections using information provided by the partner district school. Once records are received, Intake will enter the dates the IEP and evaluation report were received and the dates of each document. Based on information received, the appropriate section A, B, C, or D will become active.

School Psychologist:

  1. Reviews evaluation information and accepts or reject it

  2. Completes the In-state Transfer Form

  3. Completes the Disabilities Screen, if needed

Area Coordinator, SSD Building Principal or designated case manager:

  1. Reviews IEP information and accepts or rejects it and communicates decision to School Psychologist

  2. Completes the Transfer Interview Form if needed

  3. Complete the Comparable Services Form if needed

Area Coordinator or SSD Building Principal:

  1. Locks the event once all steps have been completed
