Exit Form

Form information

The Exit Form is completed by the Student Data (SIS) Secretaries or other assigned staff. The form documents the reason the student is not actively receiving special education services.

In certain scenarios, such as a student being found ineligible for a disability through an initial evaluation or reevaluation, the Phoenix system generates an Exit Event.  In this case the student is exited when the Initial Eligibility or Reevaluation is locked. The Exit Event serves only as a record of the student's exit.

In other scenarios, such as the student moving outside of St. Louis County or graduating, the Case Manager notifies the Student Data (SIS) Secretary who creates and completes the event. The student is exited when the Exit Event is locked.

  1. The Exit Date auto-fills from the scheduled date of the event.  The Exit Date is the date the student was determined to be “exited”.  This may be the last date the student attended school, the date the student graduated or the date the parent refused the evaluation.

  2. Select the appropriate exit reason section, then choose the specific reason for the exit (for example, “Graduated - Credits”).

  4. The Exit form is the only required form in the event.  Once saved, return to the event's overview, run Validation Check and the event can be locked.

Once the student has been exited, either after being found ineligible or locking the Exit event, the student can only return to an active status in Phoenix through an initial referral or a transfer.

The Exit form may be printed, but is for internal use only and should not be sent outside the district.
