This article is to help users with the Areas to be Assessed form.

Form information

The Areas to be Assessed form is used along with the Prior Written Notice-Additional Data located in the Initial Joint Review/Consent, Initial Eligibility and Reevaluation events if it is determined that an area(s) needs to be assessed during the evaluation process.

1. Enter the Date. The date should be the same as the date of the first attempt on PWN - Additional Data form.

2. Check box for each area to be assessed and indicate instrument(s) to be used, if known.

3. Save

4. Print the form to send to the Educational Decision Maker (EDM) along with the PWN-Additional Data form.
