This article is to help users with the Areas to be Assessed form.

Form information

The Areas to be Assessed form is used along with the Prior Written Notice-Additional Data located in the Initial Joint Review/Consent, Initial Eligibility and Reevaluation events if it is determined that additional areas need to be assessed during the evaluation process.

  1. Enter the Date. The date should be the same as the date of the first attempt on PWN.

  2. Check boxes for areas to be assessed and indicate instruments to be used, if known.

  3. Save

  4. Print the form to send to the Educational Decision Maker (EDM) with PWN-Additional Data.
