Disabilities Screen

The Eligibility section can be found in events where eligibility for special education is determined, such an Initial Eligibility or a Reevaluation Event. The Disabilities screen is in this section. 

Information

The Disabilities screen has 2 sections-Disabilities Selection and Plan Disabilities. In Phoenix, an evaluation event is referred as a “plan”. The disability, once selected, must be added to the plan before details are added.

Adding a Disability

  1. Open Disabilities  from the event overview

  2. Click the drop-down arrow to the right to open the Disabilities Selection section

  3. Select one or more disabilities

  4. Choose “+Add to Plan

  5. In the Plan Disabilities section, select a Disability Status

  6. Choose an Eligibility Date

  7. Save

  • Repeat steps 1-7 to add more disabilities

  • Use the cross icon to change the order of disabilities in the Plan Disabilities section

  • Delete a disability by selecting the trashcan for the disability in the Plan Disabilities section. This should only be done if the disability was accidentally added during the time the event is open. This is different than when a student no longer qualifies for an eligibility.

When a new disability is saved in an event, the Eligibility Determination form for the disability is generated. If the form does not generate, select the Reload Event Data to refresh the data in event which will generate forms needed such as the Eligibility Determination form.
