  1. The Behavior Intervention Plan (BIP) form is created by answering Yes to the question, “Does the student exhibit behaviors that impede his/her learning or that of others?” on the Special Considerations form, then selecting Behavior Intervention Plan.

  2. Complete all required fields. (Download the BIP Supplemental Information Guide for guidance on completing this form.)

  3. Save.

If this form has been created, completed, and saved but is no longer needed, it can be reset so it is not included in the final, locked IEP. Instructions for resetting forms are found on the Resetting Forms page.
