Behavior Intervention Plan Form

Form information

  1. The Behavior Intervention Plan (BIP) form is created by answering Yes to the question, “Does the student exhibit behaviors that impede his/her learning or that of others” on the Special Considerations form, then selecting Behavior Intervention Plan.

  2. Complete all required fields (for guidance on how to complete this form, download the BIP Supplemental Information Guide).

  3. Save.

If this form has been created, completed and saved but is no longer needed, it can be reset so it is not included in the final, locked IEP. Instructions for resetting forms are found on the Resetting Forms page.
