Form information
The Evaluation Results and Interpretation Form is used as part of an evaluation report.
Select the assessed area to make the expanding text box active.
Copy and paste the summary of the evaluation results for each area assessed. Any areas not assessed will not print in the final PDF version of the report.

Each Evaluator will enter their own evaluation summaries into Phoenix. Evaluators that assessed an area as part of the evaluation are expected to enter their interpretations into Phoenix prior to the eligibility conference.
