ESY - Documenting ESY Transportation

This article assists users when documenting ESY transportation information.

Information

This process is followed for all students requiring transportation for ESY whether they do or do not receive transportation as a related service during the school year.

Create the Transportation Change Event

  1. Open the student’s Profile in Phoenix.

  2. Click Events.

  3. Click + New Meeting in upper right corner of the Scheduled Events panel.

  4. In the Schedule Meeting window, choose Transportation Change from the Event Type menu.

  5. Enter the current date as the Due Date and Scheduled Date.

  6. Enter ESY Transportation in the Event Title Detail field.

  7. Click the smile icon to assign the event to yourself.

  8. Save.

  9. The Event appears on the Scheduled Events Panel.

Complete the Transportation Change Event

  1. Open the event.

  2. Click Transportation on the Forms List panel or the Event Overview.

  3. Complete required fields.

    1. If the student is currently receiving transportation, some fields will be completed with the most recent information and others will be blank.

  4. Enter Effective Date (first day of ESY) and End Date (last day of ESY).

  5. Check all prefilled information for accuracy and make any necessary changes.

  6. Enter the student’s Disability.

  7. In the Action section choose Add.

  8. For Session choose ESY.

  9. Choose Day(s) of the Week transportation will be needed.

  10. Ensure the pickup and drop off locations are correct for ESY.

  11. Enter Emergency Contact information for both AM and PM.

  12. Enter the email address of your SSD Administrator’s Secretary.

  13. Enter any necessary Special Instructions.

    1. If the student’s start and end times for ESY are different than the regular program times, enter the correct information in Special Instructions.

  14. Save.

  15. Click Validation Check on the form or on the Event Overview.

  16. If there are errors, address them and check validation until the event is compliant.

  17. When the Validation Check is successful, click Lock.

  18. When the Transportation Change event has been locked your SSD Administrator’s Secretary will receive an email informing her that the form has been completed. She will complete the process.

Transportation accommodations do not appear on the Transportation Form. If the accommodations need to be updated an Amendment or new IEP must be completed.
