Front Page - Services Plan

This article will help users with the form.

Form information

  1. Click Front Page-Services Plan tab (all dates on the Plan dates page auto-fill from the Notification of Meeting and should only be changed if the meeting date changes).

  2. Enter necessary information.  Contact the SNAP department to make changes to the information that is auto-filled in green boxes.

  3. Verify the accuracy of the Non-Public school.

  4. Enter SNAP site in Location for Provision of Service.

  5. Enter date SNAP secretary is contacted to mail copies of the Services Plan document (the date should be no more than 10 days from the date of the Services Plan meeting).

  6. Enter the mode of delivery.

  7. Select the Educational Decision Maker.

  8. Enter the Name of the Educational Decision Maker.

  9. Select the appropriate email option.

    1. If the parent provided an email address, enter the address in the Email field. The email address will print on the Front Page.

    2. If the parent declined to provide an email address, enter the reason the parent declined. The reason does not print.

    3. If the parent has no email address, select "Parent has no email address".

  10. The Date of the most recent evaluation/reevaluation is autofilled based on the last, locked Eligibility or Reevaluation Event at the time the SP is opened for the first time.  If a reevaluation with no further assessment or the Triennial Evaluation Documentation Form is completed with this SP, change this date to the date of the SP.

  11. If the Projected date for next triennial evaluation does not autofill or is incorrect, enter the date of the most recent evaluation/reevaluation plus 3 years.

  12. Enter the names of participants in the meeting and their Title.

  13. Click the Save button.
