Form information
The Non-Resident Services Plan Decision form is included in the Non-Resident Services Plan event which is completed by the SEIMS team at the Service Desk. It is used to document the student's Non-Resident Services Plan status (is or is no longer receiving services through Non-Resident Services Plan). It is only editable by the Service Desk staff. It is a view-only form for all other users.
The Non-Resident Services Plan event, which includes the Non-Resident Services Plan Decision Form, is completed by the SEIMS team at the Service Desk.

The Nonresident Services Plan is not completed in Phoenix. It is a fillable PDF which is downloaded from the Non-Resident Services Plan Information and Forms page.
