Front Page Additional Participants

This article assists users when completing the form.

Form information

The Front Page Additional Participants form is used when additional space is needed to document participants in an IEP Meeting. It is created when the box at the bottom of the IEP Front Page is checked. 

  1. Enter the name and method of attendance for each participant.

  2. Save.

If this form has been created, completed, and saved but is no longer needed, it can be reset so it is not included in the final, locked IEP. Instructions for resetting forms are found on the Resetting Forms page.
