Form D-Part 3: Alternate Assessment (MAP-A)
This article assists users when completing the form.
Form information
Form D-Part 3 documents the student’s participation in MAP-A and the justification for the student not participating in the regular assessment (GLA, EOC, or NAEP). The form is created when MAP-A is selected in the State Assessments section of the Special Considerations form.
This form is used only when the student is participating in MAP-A. If the student participates in the regular assessment, return to Special Considerations, uncheck the option for MAP-A, and then select the assessment(s) in which the student will participate. This will create the appropriate section(s) of Form D.
Select the area or areas in which the student will participate in MAP-A.
Complete the section for Dynamic Learning Maps.
Save.
Once you’ve completed Form D-Part 3: Alternate Assessment (MAP-A), review the PLAAFP to ensure the final questions regarding MAP-A have been answered appropriately.
NOTE: If MAP-A appears on a report but MAP-A is not provided for the student, the denotations for Forms D on the Special Considerations page will need to be updated. Form D part 3 may also need to be reset. (See below)
If this form has been created, completed, and saved but is no longer needed, it can be reset so it is not included in the final, locked IEP or Amendment. Instructions for resetting forms are found on the Resetting Forms page.
