Form D-Part 1: Grade-Level Assessments

This article assists users when completing the form.

Form information

Form D-Part 1 documents the student’s participation in Grade Level Assessments. The form is created when Grade-Level Assessment is selected in the State Assessments section of the Special Considerations form.

If the student participates in MAP-A, this form will not be used. If it was created in error, return to Special Considerations, uncheck the option for Grade-Level Assessments,, then select MAP-A. This will create Form D-Part 3 to document participation in MAP-A.

A copy of DESE’s Form D can be downloaded here. It includes the accommodations for the Grade-Level Assessments, which can be used for reference. The SSD form may vary slightly from the DESE form.

  1. Select how the student will participate in the Grade Level Assessment - WITHOUT or WITH Accommodations.

  2. In Sections B and C, indicate if the IEP team has recommended Universal Tools in that section.*

    1. If Yes is selected, select the Universal Tools recommended by the team.

  3. If the student will participate with accommodations, the accommodations are entered on the Assessments screen. The assessment areas and accommodations print on Form D-Part 1.

*Consult with your SSD Administrator for guidance on completing the Universal Tools sections.
