Review of Existing Data Form - Team Conclusions & Decisions

Form information

The RED-Team Conclusions and Decisions Page is completed to document decisions made through the review of existing data.  For an initial evaluation, it is completed by the School Psychologist.  For a Reevaluation, it is completed by the Case Manager.  Depending on the option selected, subsequent sections of the form will become active. Therefore, it is imperative that you are making the correct decisions. Making an incorrect choice can result in exiting the student. Please see more information below on choosing the correction option for a reevaluation with no additional data.


Additional Data is Needed

  1. Select Additional data is needed

  2. Select Initial Evaluation

  3. Enter the date the decision was finalized

  4. Enter the names of those participating in making the decision

  5. Save

Reevaluation

  1. Select Additional data is needed

  2. Select Reevaluation

  3. Enter the date the decision was finalized

  4. Enter the names of those participating in making the decision

  5. Save

  6. Do not lock the Reevaluation Event

  7. Notify the Evaluation Chair within 3 days of the Review of Existing Data meeting that additional data has been requested

  • The Evaluation Chair will review the form, obtain consent, and coordinate the completion of the Reevaluation and will lock it

  • The Prior Written Notice-Additional Data and the Areas to Be Assessed forms can only be completed by Evaluation Chairs. The forms are "view only" for Special Education teachers.


No Additional Data is Needed

  1. Select No additional data is needed. Choose type of evaluation:

  2. Select Initial Evaluation or Reevaluation

  3. If this is a Reevaluation, select the appropriate reason based on the team's decision. Be sure to make your selection carefully. Choosing the incorrect option could result in the student being exited in error.

  4. Enter the date the decision was finalized

  5. Enter the names of those participating in making the decision

  6. Save
