Discipline Documentation Form - Section A

This article assists users when completing the form.

Form information

Section A of the Discipline Documentation form is created in the Disciplinary Documentation event when Page 1 is completed, indicating the student has been suspended for more than 10 days in a school year, but no pattern has been created.

  1. Click the box next to School personnelin consultation with at least one of the child’s teachers, determines the extent to what services are required on the 11th school day and thereafter. . .

  2. Enter each of the factors considered in determining the extent to which services are required, including the length of removal, the extent to which the student has been removed previously, and the student’s needs and educational goals.

  3. Choose the Decision option: Services are NOT required, or Services ARE required.

  4. Complete the required field based on the answer to the previous question.

  5. Enter the Date of the Decision.

  6. Enter the Names and roles of the individuals making the decision.

  7. Save.

  8. Return to the Event Overview.

  9. Click the Validation Check button.

  10. If errors appear, resolve them before continuing.

  11. Click Print to preview and proofread Page 1 and Section A.

  12. When the forms are correct, Lock the event.

  13. No additional events are created.  The process is complete.

If this form has been created, completed, and saved but is no longer needed, it can be reset so it is not included in the final, locked event. Instructions for resetting forms are found on the Resetting Forms page.
