Additional Information Form
The Additional Information form can be used to document additional information pertaining to an event. The form contains an expanding text box that will allow for multiple pages of the form.

An Additional Information form is included in the Manifestation Determination Event. This required form is included to summarize the final outcome of the disciplinary action taken by the school/district including the number of suspension days ultimately given for this infraction as well as the total days the student has been suspended this school year. Other outcomes that may be included, as appropriate, include: IEP/Amendment scheduled or completed, reevaluation requested, etc.
All members of the IEP team will enter Present Level information on the Present Level of Academic Achievement and Functional Progress. The Additional Information page is not used for this purpose.
Comparable Services are no longer documented on the Additional Information form in the Transfer Event. If comparable services are to be provided they are documented on the Comparable Services Form.
