Therapy Log assistance
This article provides a reference for technicians when attempting to resolve issues customers may experience using Therapy Log, as well as general information such as providing a contact for different support scenarios.
What is Therapy Log?
It is a web based system for Administrators and Support Staff to track any contact with students. Some contacts entered into the system can result in reimbursement from Medicaid.
Who uses Therapy Log?
Use at SSD is currently limited to: Private Duty Nurses (1 to 1), Social Workers, SLP’s (Speech Language Pathologists), OT’s, and PT’s (Occupational and Physical Therapists) as well as their supervisors.
How do staff at SSD get enrolled with an account for Therapy Log?
A direct supervisor alerts Collin Swearingen at MSBA Medicaid Consortium that a new account is needed and Collin sets up the account. His system auto generates a password which is shared with the staff member once the account is generated, to their SSD email.

How does a student record appear in Therapy Log from Phoenix?
The following criteria must be met for a student's record in Phoenix to be loaded into Therapy Log:
On the Summary page in Phoenix, the student must have a Program Status of "Active (Special Ed)"
The student must have a current, active plan in Phoenix
The student must have at least one of the following active services:
Special Education Services-Speech Therapy or Language Therapy
Related Services-PT, SP, OT, LT and Social Work Counseling
Consult with Social Worker
Social Work Services
At least one of the services must be assigned to a licensed, MO HealthNet enrolled service provider
Must have at least one active goal associated with one of the services listed above
