Form for temporary staff to fill out to obtain access to District technology resources and accounts. To be completed before account will be created and access granted.

Please have contractors, agency staff, and other temporarily assigned non-SSD staff that are consulting, subbing, or otherwise working on a temporary basis that need access to District technology systems and hardware complete the attached form above.
