This article assists users when completing the form.
Form information
The Record of Attempts Form documents the district's attempts to schedule a meeting. It is included in all events with a Notification of Meeting form.
Enter the Date of contact.
If the parent waived the 10 day notification requirement, check the box.
Check appropriate Method(s) of contact. More than one may be chosen.
Enter the Parent/Guardian Response. If "Cannot attend . . ." or "No response . . ." is chosen, the 2nd attempt is required.
Click the Save button.
If needed, complete the required fields in the 2nd Attempt section.
Once the Notification of Meeting is compliant and Create/Store invite selected, the Print Layout Chooser appears on the screen. The Notice of Meeting and Record of Attempts are both included on the drop down, however, only the Notification of Meeting is printed and sent to the parent. The Record of Attempts is for district documentation of the attempts to schedule the meeting and is not provided to parents or partner district staff with the Notice of Meeting. It is for internal use only.
If the meeting is rescheduled, complete the Notification of Meeting with the new date, time, etc. then reopen the Record of Attempts. In the Parent Response section for the 1st Attempt, choose either Cannot attend . . . or No response . . . to make the 2nd Attempt section required.
The Notification of Meeting and Record of Attempts are stored in the Invitation Record when Create/Store Invite is clicked. If information is changed on either the Notification of Meeting or the Record of Attempts after the Notification has been Stored (Create/Store Invite was clicked), the forms must be Saved, AND Create/Store Invite must also be selected to lock and store the most recent version of the forms. If this is not done, the most recent version of the forms will not be included with the locked event.
Reevaluations
When further assessment has been requested through the Review of Existing Data for a Reevaluation, two Notifications of Meeting will be sent to the parents. Two separate Record of Attempt forms must also be completed to document the two Notifications of Meeting, one for the Review of Existing Data (sent by IEP Case Manager) and one for the Eligibility Conference (sent by Evaluation Chair).
If a meeting is needed to review evaluation information, the Evaluation Chair completes the following steps in conjunction with the additional Notification of Meeting:
Open the Notification of Meeting.
At the bottom of the Event Overview, select Invitation Record to ensure there is a saved version of the NOM from the RED meeting. If there is not, contact the Case Manager who will complete it.
When the NOM for the RED meeting has been stored, complete the NOM and Record of Attempts for the Eligibility Conference.
If the Case Manager used both the First and Second Attempts, change the selection in the Parent Response section to one of the options that does not require a Second Attempt (Do not want to attend or Yes, I'll be there). This will clear the Second Attempt.
Proceed with documenting the attempts to schedule the meeting to discuss the evaluation results.
Enter the Date of contact.

Enter the Parent/Guardian Response. If Cannot attend . . . or No response . . . is chosen, the 2nd Attempt will become required.
Click the Save button.
Click Create/Store Invite.
Select First Attempt from the drop down if it is the first attempt to schedule the Eligibility Conference.
Click the Save button.
