This article will help the user with the form.

Form information

The Reevaluation Decision Form is used to indicate the decision whether a Reevaluation is or is not necessary. The choice made on this form determines whether the Triennial Evaluation Document is created in this event OR the Reevaluation Event which includes the Reviewing of Existing Data form is created when this event is locked. The form is completed by the IEP case manager.

Two options are available:

  1. If it is determined that a reevaluation is not necessary, the first option is selected which creates the Triennial Evaluation Documentation form as required in this event. In this scenario, no Review of Existing Data form is created or completed OR

  2. If it determined that a reevaluation is necessary, the second option is selected.  After the Reevaluation Decision event is locked, a Reevaluation Event is created where a Review of Existing Data can be completed.

  3. Save

The Review of Existing Data Form is located in the Reevaluation Event. That event is created when the Reevaluation Decision form indicates that a reevaluation is necessary and the Reevaluation Decision Event is locked.
