ESY - Documenting ESY Transportation in an IEP

This article assists users when documenting ESY transportation information in an IEP.

Information

This process is followed for students who currently receive transportation as a related service as well as those students who will be receiving transportation only for ESY.

Create the Transportation Change Event

  1. Open the student’s record in Phoenix

  2. Click Events

  3. Click + New Meeting in upper right corner of the Scheduled Events panel

  4. In the Schedule Meeting window, choose Transportation Change from the Event Type menu

  5. Enter the current date as the Due Date and Scheduled Date

  6. Enter “ESY Transportation” in the Event Title Detail field

  7. Click the smile icon to assign the event to yourself

  8. Click Save

  9. The Event will appear on the Scheduled Events Panel

Complete the Transportation Change Event

  1. Open the event

  2. Click Transportation on the Forms List panel or the Event Overview

  3. Complete required fields

  4. Enter Effective Date (first day of ESY) and End Date (last day of ESY)

  5. Check all prefilled information for accuracy and make any necessary changes

  6. Enter the student’s Disability

  7. In the Action section choose Add

  8. For Session choose ESY

  9. Choose Day(s) of the Week transportation will be used

  10. Ensure that the pickup and drop off locations are correct for ESY

  11. Enter Emergency Contact information for both AM and PM

  12. Enter the email address of your SSD Administrator’s Secretary

  13. Enter any necessary Special Instructions. If the student’s start and end times for ESY are different than the regular program times, enter the correct information in Special Instructions.

  14. Save

  15. Click Event Overview on the Event Sections panel

  16. Click Validation Check

  17. If there are errors, address them and check validation until the event is compliant

  18. When the event is compliant, click Lock

  19. When the Transportation Change Event has been locked your SSD Administrator’s Secretary will receive an email informing her that the form has been completed. She will complete the process.

Transportation accommodations do not appear on the Transportation Form. If the accommodations need to be updated an Amendment or new IEP must be completed.
