The Non-Resident Services Plan Decision Form is included in the Non-Resident Services Plan Event which is completed by the Service Desk. It is used to document the student's Non-Resident Services Plan status (is or is no longer receiving services through Non-Resident Services Plan). It is only editable by the Service Desk staff. It is a view-only form for all other users.
