Disciplinary Documentation Section E is included in the Manifestation Determination event. It must be completed if the suspension is for a violation involving drugs/weapons/serious bodily injury for any number of days (it does not have to be 45 days).
Check each of the first 2 boxes – the parent was notified of the decision and the parent was provided a copy of Procedural Safeguards.
Check the box for Conduct a Manifestation Determination.
Check the appropriate box indicating whether the conduct is or is not related to the disability.
