This article will help users when completing the form.

Form information

The PWN-No Consent Needed is available in the Initial Joint Review/Consent, Initial Eligibility and Reevaluation Events. The following scenarios demonstrate when the form is needed as part of the event.

Initial Joint Review/Consent Event

If during a parent referral, Evaluation Not Warranted is selected on the Referral for Evaluation Form, the PWN-No Consent Needed will appear as a required form within the IJR Consent Event.

Initial Eligibility Event

When the student is determined to be eligible or ineligible for special education services, the PWN-No Consent Needed is required in the Initial Eligibility Event. 

Reevaluation Event

If the team determines the student is ineligible or there is a change in the disability either through a Review of Existing Data or through a Reevaluation with testing, the PWN-No Consent Needed is available as a form within the Review of Existing Data/PWN Event Section. 


Completing the PWN - No Consent Needed form:

The PWN- No Consent Needed form is available in the event.

  1. Indicate the method in which the Prior Written Notice was presented

  2. Enter Date of Notice

  3. Choose Proposed  or Refused by our district and choose an action from the drop-down

  4. Enter the Description and explanation of the action

  5. Enter the Options considered and why rejected, referring to your department templates

  6. Enter what the action was based on

  7. Enter Other relevant factors considered

  8. Enter Evaluation Chair contact information

  9. Save and Print the form

PWN-No Consent Needed-2 is available within the Reevaluation Event if the PWN-No Consent Needed within the Review of Existing Data/PWN Event Section has been used and assessment was needed in additional areas.

If the Prior Written Notice-No Consent Needed form is optional but is completed in Phoenix and later found to be not needed, it can be reset.  After saving, click Set Status to Blank at the bottom of the screen.  This will change the form status to a blank form and it will not be included in validation checks and will not be included when the event is locked.
