This article assists users when completing the form.
Form information
The purpose of this form is to ensure Alternate Form F prints as part of the IEP.
The form is created when 'Documented on Alternate Form F' is selected under Program Modifications and Accommodations on the Services Summary form.
Select the radio button for Include Alternate Form F in print layout.
Click the Save button.
If Documented on Alternate Form F is not selected, the form will not be created and Alternate Form F will not appear in the printed IEP, even if accommodations are chosen on the Classroom Accommodations Screen. If this happens it will be necessary to complete an IEP Correction.
If this form has been created, completed and saved but is no longer needed, it can be reset so it is not included in the final, locked IEP. Instructions for resetting forms are found on the Resetting Forms page.
