Disciplinary Documentation Section E is included in the Manifestation Determination event. It must be completed if the suspension is for a violation involving drugs/weapons/serious bodily injury for any number of days (it does not have to be 45 days).
Check each of the boxes – the parent was notified of the decision,the parent was provided a copy of Procedural Safeguards, and aManifestation Determination was conducted.
Make the appropriate selection indicating whether the conduct was or was not related to the disability.
