This article assists users when completing the event.
The IEP Correction event is created by the user. It is used to correct an error in a locked IEP.
To change a decision made in an IEP create an IEP Amendment event.
If a mistake is made in an IEP Amendment event, create a new IEP Amendment event, not an IEP Correction. The Amendment form is not included in the Correction event but is required when completing an amendment.

Important information about completing an IEP Correction event following the September 15, 2021 Phoenix update:
On Special Considerations, the State Assessments and Alternate Method of Instruction sections will be blank and must be completed. The other sections copy from the previous event.
If ‘YES’ is selected in the new State Assessments section, the assessment(s) in which the student will participate must be selected. These selections create the ‘parts’ of Form D which will be required. (Part 1: Grade Level Assessments, Part 2: EOC, Part 3: MAP-A, Part 4: ACCESS, Part 5: NAEP).
The selected ‘parts’ of Form D are required and must be completed, including the Universal Tools sections. Please contact your SSD Administrator for guidance about completing the universals sections.
If accommodations are needed for state assessments, they must be reentered on the Assessments screen using the new accommodations.
If, when locking the event, error messages appear for the old sections of Form D, return to the Event Overview and select ‘Reset Optional Forms’ at the bottom. Select the page(s) that appeared on the error messages, and Save. Please contact the Service Desk for assistance, if needed.

What is an IEP Correction?
The IEP Correction is used to correct a clerical error discovered after an IEP is locked. It cannot be used to change decisions made at the IEP meeting, only to fix a mistake made when entering the decision in the event. It is not used to correct errors made in an IEP Amendment.
What forms are included in an IEP Correction?
When an IEP Correction is created all forms in the IEP being corrected are copied including the Excusal, IEP Front Page, Front Page-Additional Participants, and Prior Written Notices.
The Correction Decision form is included to determine if a Correction Notification form is needed. The Notification form is created if the parent has received a copy of the original IEP.
The Amendment Form is not included in the IEP Correction event. If a mistake is made in an Amendment, a new IEP Amendment event must be created.
