Therapy Log-Criteria Needed for Student Record to Appear

The information below provides the action needed for a student to be imported into TherapyLog from Phoenix and the responsible staff for providing that information. When troubleshooting issues, please refer to this information before contacting the Service Desk.

Student is “ACTIVE” on ESI Status:

  1. Search for student

  2. ESI Status is “ACTIVE” in header  

A student’s record is ACTIVE in ESI the next business day after the first event (Initial Joint Review or Transfer event) has been completed by the Primary Staff assigned to it in Phoenix (based on most recent enrollment)

Phoenix Status is “ACTIVE (SPECIAL ED), ENROLLED” or “ACTIVE (NON-RESIDENT SERVICES PLAN), ENROLLED” in header  

ACTIVE (SPECIAL ED) indicates the Case Manager has locked an active plan in Phoenix

ENROLLED indicates the student has been enrolled by the Intake Department in ESI and added to Phoenix the next business day

If student is SNAP, must have SNAP indicator on student code

  1. Search for student

  2. Check for “SNAP Indicator” on Student Codes tab

SNAP Department

If student is NRSP, must have NRSP indicator on student code

  1. Search for student

  2. Check for “Non-Resident Services Plan” on “Student Codes tab

Student Data

PHOENIX

Student requires an active (special ed) status:

  1. Search for student

  2. Program Status is “Active (Special Ed)” on Summary screen

The Case Manager has locked an active plan

PHOENIX

Student must have an active plan in Phoenix

The Case manager has locked an active plan

PHOENIX

Must have at least one Medicaid-eligible service, service minutes greater than zero with a start date that has started and end date has not passed

  • Speech Therapy

  • Language Therapy

  • Related Services – Physical Therapy

  • Related Services – Speech Therapy

  • Related Services – Occupational Therapy

  • Related Services – Language Therapy

  • Related Services – Social Work Counseling

  • Consult with Social Worker

  • Inst in Social Skills

The case manager/IEP team member has entered the service in the current plan

PHOENIX

Must have at least one service assigned to a licensed, MO HealthNet enrolled service provider

The case manager/IEP team member has assigned the service to provider in current plan

PHOENIX

Must have an active goal associated with the eligible service

The case manager/IEP team member has written a goal associated with eligible service in current plan
