The Evaluation Summary form is intended to be an “at-a-glance” list of areas of disability for which the student was determined to be eligible. Sections of this form will be required based on information from the Disabilities screen.
Indicate if a student is not eligible or is eligible for a disability.
If eligible, select disability(ies).
The Areas of Disability will fill in after they are selected on the Eligibility Determination Form(s).
An expanding text box is available at the bottom of the form (soft grey outline) which will be used to summarize information indicating areas/disabilities that were discussed at the conference and the team determined that the student was ineligible.
