Form F is not completed as a form in Phoenix. It is included in the printed IEP when Documented on Alternate Form F is selected on the Services Summary form and the Alternate Form F Decision form is completed. The accommodations selected on the Classroom Accommodations screen print on Alternate Form F.

Reminder
If Documented on Alternate Form F is not selected on Services Summary, Form F will not be created or appear in the printed IEP, even if accommodations are chosen on the Classroom Accommodations Screen. If this happens, and the event is locked, it is necessary to complete an IEP Correction. Careful proofreading before locking will prevent this.
