Creating an IEP Amendment or IEP Correction Event

This article is to assist users when creating the IEP Amendment or IEP Correction event. Instructions for creating other user created events can be found here. Instructions for creating the IEP Amendment or IEP Correction event can also be found on the Amendment/Correction Quick Start.


When should an IEP Amendment be created?

An IEP Amendment is used to document a change to a decision made by the IEP team at the IEP meeting or in a subsequent amendment. Examples are changing the location of a service from the Special Education setting to the Regular Education setting and adding new classroom or assessment accommodations.


When should an IEP Correction be created?

An IEP Correction is used to correct an error (i.e. a clerical error) discovered in an IEP after it is locked. An example would be a service or goal agreed to at the meeting but not entered prior to locking the IEP.

An IEP Correction is NOT used to correct an error in an IEP Amendment. In this case a new IEP Amendment event should be created to correct the error.


What should I do to correct an error in an IEP Amendment?

If an error is made in an IEP Amendment event, create a new IEP Amendment event to make the correction.

The Amendment form is not included in the IEP Correction event but is required when completing an Amendment. An IEP Correction event completed following an Amendment will not include the Amendment form or the IEP Front Page and Excusal which were not in the Amendment copied into the Correction event.


Can an IEP Amendment or IEP Correction be created if the IEP is in PENDING ACTIVE status?

Yes.

An IEP in PENDING ACTIVE status can be amended or corrected. “PENDING ACTIVE” means the Initiation Date is in the future. The ability to correct a "PENDING ACTIVE IEP means an incorrect Initiation Date can be corrected immediately.  

When an IEP Correction or IEP Amendment is locked for an IEP, IEP Amendment, or IEP Correction in PENDING ACTIVE status, the status of the original event will change from PENDING ACTIVE to ABANDONED. This is because the original event never became active and was never implemented.


Can an IEP Amendment or IEP Correction be created if the IEP is in PENDING status?

No.

An IEP is in PENDING status because the Initial IEP has been locked but the Parent Consent-Initial Services is not locked. An amendment or correction cannot be created until the Parent Consent-Initial Services is locked moving the IEP to either PENDINGACTIVE or ACTIVE status (depending on the Initiation Date).


Can an IEP Amendment or IEP Correction be created if the IEP has expired?

No.

An expired IEP cannot be amended or corrected. The Plan Status of an expired IEP is shown on the Locked Events panel with red fill. The plan is still “Active” in Phoenix but there are no active goals, services or accommodations.


Creating the Events

  1. Open the student's profile.

  2. On the student's profile, click the Events link.

  3. On the student's Events panel click + New Meeting.

  4. On the Schedule Meeting window, choose IEP Amendment or IEP Correction from the Event Type menu. In this example an IEP Amendment will be selected.

  5. Select the Plan to be Amended. In most cases there will only be one option. If the student has both a Pending Active Plan (IEP, IEP Amendment, or IEP Correction) and an Active Plan there will be two. The date is the Meeting Date of the original IEP being amended or corrected even if other amendments or corrections have been completed.

  6. If the "Student School" field is visible below the Event Type, choose the school the student currently attends from the menu. This field is not visible for all students. 

  7. The Scheduled Date and the Due Date fill in automatically and cannot be changed.

    1. The Scheduled Date is the Meeting Date of the original IEP being amended or corrected even if other amendments or corrections have been completed. An Amendment does not change the IEP Meeting Date.

    2. The Due Date is ten days from the date the event is created.

  8. A default “Scheduled Time” is entered. It is not necessary to change it as there is not usually a meeting for this event.

  9. The Event Details field can be used to enter the reason for the amendment or correction if desired. The information entered becomes a part of the event name. It can be used to specify the reason for the amendment or correction but is not required.

  10. Click the Smile icon to assign the event to yourself or the Search icon to open the Staff Search window allowing you to search for and select another user.

  11. Save.

  12. The IEP Amendment or IEP Correction event appears on the Scheduled Events panel. An identifier appears on Scheduled Events and Locked Events to show the connection of the new event to the locked event it is modifying. For an amendment it is two arrows. For a correction it is a check mark. The identifier is not included after the event is locked.

  13. The Scheduled Date of the IEP Amendment or IEP Correction has red fill because the date is in the past. This is correct because the event being amended or corrected occurred in the past.


Reminder

All IEP Amendment and IEP Correction events must be completed and locked, or deleted, before a new IEP is locked. Contact the SSD Help Center  to have a user created event deleted if it is not needed.
