This article assists users when completing the form.

Form information

  1. The Transfer of Rights form is created when the box for Form is needed for this IEP is checked in the Transfer of Rights section of the Special Considerations form. It is also included in the Transfer of Rights event.

  2. The date in the top left corner of the form is either the date of the IEP meeting or the date entered as the Scheduled Date of the event if the form was created outside of an IEP.  It is not editable.

  3. Choose the Mode of Delivery for the student from the drop down.

  4. Enter the name of the student’s Parent/Guardian and choose the Mode of Delivery from the drop down.

  5. Enter the name of the Educational Decision Maker.  Before the student is 18 this is usually the parent/guardian but can be different.  Be sure to check carefully. The student becomes the EDM at age 18.

  6. The student’s name and birth date auto-fill.

  7. Enter the date of the student’s 18th birthday.

  8. Complete the form entering your contact information and name.

  9. Save.

The Transfer of Rights form is issued twice:  the first at least one year before the student turns 18 (at 16) and again when the student turns 18.

If this form has been created, completed and saved but is no longer needed, it can be reset so it is not included in the final, locked IEP or Amendment. Instructions for resetting forms are found on the Resetting Forms page.

The Transfer of Rights form can also be created as part of a user created event if it is needed outside of an IEP. Instructions can be found here.
