Form information
The Transfer Interview form is completed by the Area Coordinator, SSD Building Principal or designated Case Manager to document the interview completed at the time the student enrolled in the Partner District if the IEP has not been received. The form is used to gather information from the parent and the previous district to assist in determining comparable services.
Complete the form based on information provided by the Partner District

If the interview indicates comparable services need to be provided to the student, those services are documented on the Comparable ServicesForm.
