How to change and/or retrieve my PDF password in Document Self-Service

This article is to help any user trying to change or retrieve their password in order to see your paycheck stubs, W2's and 1095C.

Forgot my password

  1. Login to the SSD Portal, https://portal.ssdmo.org,  with your SSD username and password.

  2. Click on the Applications tab at the bottom of the screen and click the Document Self-Service application.

  3. Login to Document Self-Service using your SSD username and password (fmlast or last.f).

  4. Click the My Delivery Settings drop-down menu located in the top navigation section.

  5. Click the Email Delivery option under PayStubs in the left navigation menu.

  6. Scroll down the page and locate the I forgot My PDF Password and the Change My PDF Password links.

  7. Click the I forgot My PDF Password link.

  8. Click the Email button.

  9. Look in your SSD email for a new message in your Inbox.


Change my password

  1. Login to Document Self-Service using your SSD username and password (fmlast or last.f).

  2. Click the My Delivery Settings drop-down menu located in the top navigation section.

  3. Click the Email Delivery option under PayStubs in the left navigation menu.

  4. Scroll down the page and locate the I forgot My PDF Password and the Change My PDF Password links.

  5. Click the Change My PDF Password link.

  6. Enter your current password.

  7. Enter your new password and retype it.

  8. Click Change button.
