Form F: Classroom Accommodations and Modifications is not completed as a form in Phoenix. It is included in the printed IEP when Documented on Form F: Classroom Accommodations and Modifications is selected on the Services Summary form and Form F: Classroom Accommodations and Modifications is completed. The accommodations selected on the Classroom Accommodations screen print on Form F: Classroom Accommodations and Modifications.

Reminder
If Documented on Form F: Classroom Accommodations and Modifications is not selected on Services Summary, Form F will not be created or appear in the printed IEP, even if accommodations are chosen on the Classroom Accommodations Screen. If this happens, and the event is locked, it is necessary to complete an IEP Correction. Careful proofreading before locking will prevent this.
