These videos for Phoenix navigation include basic navigation information, completing an IEP and other tasks. The individual videos are from four to 12 minutes long.

Key Points to Remember After Training

Initial IEPs

Initial IEPs

  • An Initial IEP will be completed for Out of State students who do not have a Missouri IEP on record.

  • An Initial IEP must also be completed for students who have recently been found eligible through an IJR/Initial Eligibility and who have never had a Missouri IEP.

  • An Initial IEP should not be locked until the PWN - Parent Consent for Initial Services has been completed AND the consent form has been signed and returned.

  • The signed PWN - Parent Consent for Initial Services should be sent to Student Records.

  • All other information regarding Initial IEPs is identical to all IEPs and can be found in the next section.

IEPs

  • IEPs are due every 364 days.

  • Students transferring into district will have either an IEP (Transfer) or an Initial IEP (see above). IEP meetings must be held for all students transferring in who do not have an accepted IEP from another district. If the IEP is accepted and NO changes or modifications will be made to the accepted IEP, goals and services must still be copied into the IEP (transfer) and that document must be locked.

  • All students MUST have an active IEP in Phoenix to be considered active SSD students.

  • Plan dates must align with the meeting date of the IEP - the IEP must initiate on or after the meeting date and end 364 days after the meeting date.

  • It is advisable to type the Present Level (PLAAFP) in Word and paste it into Phoenix. To paste, use CTRL+Shift+V (or CMMD+Shift+V)

  • All IEPs must have at least one Special Education Service before they can be locked.

  • When holding a new IEP, ensure that there is not an open IEP amendment or IEP Correction. If there is either lock the IEP Amendment or IEP Correction or contact Technology Services and request that event be deleted prior to locking the new IEP.

  • Proof read, proof read and proof read again before locking.

  • If the IEP is locked with a future initiation date, it will show as PENDING ACTIVE until the initiation date. If the plan should be ACTIVE, an IEP correction can be done to adjust the start and initiation dates on the Plan Dates form.

  • On the 365th day after the IEP meeting was held, Services and Goals will no longer be active, no new progress can be created nor will be generated for that IEP, Amendments and Corrections cannot be created or locked, and the student will no longer show in provider lists. The locked IEP will show a plan status of ACTIVE, but the word will appear in red, which indicates it is the most recent IEP but is no longer current.

 Corrections/Amendments

  • If an IEP is locked and is then found to have clerical errors, an IEP correction can be completed.

  • If an IEP is locked and modifications are later needed OR if an Amendment has already been completed in the IEP cycle, an IEP Amendment should be completed.

Reevaluations

  • The event automatically generates due 3 years from the previous evaluation event. The Reevaluation Decision is due 105 days prior to the Reevaluation due date to provide time to Evaluation Chairs to complete any testing.

  • The IEP chair completes the NOM for the Review of Existing Data meeting.

  • The IEP chair must notify the Evaluation Chair within three days of a decision for further testing.

  • IEP Chairs do NOT lock Reevaluations if there is a change in eligibility (dropping or adding a disability) unless they also serve as Evaluation Chair (ie they are Speech Language Pathologists or Occupational or Physical Therapists).

Progress Reports

  • Progress reports are auto-generated by Phoenix based on dates set by SSD Directors.

  • Progress generates 10 days prior to the due date.

  • To complete progress, always click ON the date of progress and select only progress for which you are goal provider. If an error occurs when marking progress complete, uncheck “Assigned to Me”. If additional progress reports then appear, fill in the date and ensure that the newly revealed progress is appropriately assigned.

  • Progress summaries can be pasted into Progress Reports. To paste, use CTRL+Shift+V (or CMMD+Shift+V)

ESY

  • If summer school determinations are made after the IEP is locked, an IEP Amendment should be completed.

  • Adding ESY requires modifications to four forms: Special Considerations, Form B, Goals, and Services.
