Form A is completed for students who are blind or who have visual impairments.
The form is created by answering Yes to the question Is the student blind or visually impaired on the Special Considerations form.
The form should be filled out by, or with input from, a Teacher of the Visually Impaired.
Complete all required fields.
Save.
If this form has been created, completed and saved but is no longer needed, it can be reset so it is not included in the final, locked IEP. Instructions for resetting forms are found on the Resetting Forms page.
