A Reevaluation that will result in a change in eligibility must be manually added to ESI Caselog by the Evaluation Chair because the event does not automatically appear as an Initial Eligibility event does. 

 Prior to adding a Reevaluation event in ESI, the following has occurred in Phoenix:

  • The RED-Team Conclusions and Decisions page is completed and indicates Additional Data is Needed or No Additional Data is Needed but there will be a change in eligibility

  • The Evaluation Chair has been notified by the IEP Chair that additional data is needed or there is a change in eligibility (including an exit) for the reevaluation

  • The Notification of Meeting and Record of Attempts forms have been created, stored and sent for the Review of Existing Data (RED) meeting

The event in Phoenix should not be locked at this point. If the event has been locked, the reevaluation can still be added to Caselog and a report can be submitted to Word Processing as long as there were no errors in the event.

Adding the Reevaluation Event to Caselog

  1. Enter the SSD# in the Search field and click the magnifying glass

  2. Click the Caselog tab

  3. Open the Add Event Process panel

  4. Click the Event Process and choose the evaluation type

  5. Select the Reevaluation in the drop-down (If the reevaluation event has been locked in Phoenix, check the box for View Locked Reevaluations to select it) 

  6. Save

An initial eligibility does not require this process. It is automatically added to ESI Caselog as a part of the Initial Joint Review process.

A reevaluation where the current eligibility will continue with no changes (completed by the IEP Chair) and RED Page 6-Parent Notification goes home to the parents will not require an entry in Caselog.
