For any student with Form D accommodations the IEP team should consider whether either of these accommodations ensure that the accommodations are appropriately updated to reflect DESE’s changes if an amendment or correction is made.

Form D Parts 1, 2, and 5

DESE has made the following changes to Form D Part 1, 2, and 5. For any student with Form D Part 1, 2, or 5 accommodations the IEP team should consider whether either of these accommodations should be added for science and add them if appropriate.

Form D part 1 and 2 additions

·        Abacus and multiplication table for grades 4-8 have been added for Science.

Form D part 5 additions/removals

·        Individual/Small groups added for math and reading

·        Small groups (removed from Section A)

DESE has made the following changes to Form D Part 4.

The following accommodations have been added as options:

  • Extended Testing Time (to Section A)

  • Read Aloud to Self (to Section A)

  • Recording Device and Transcription

Form D part 4 Accommodations Replaced – Changes REQUIRED if an Amendment or Correction is completed

The following accommodations have been replaced. If a student had any of the following and an amendment or correction is completed, Form D Part 4 MUST be reviewed, and the new accommodation added when necessary:

  • Scribe - replaced with Scribed ResponseBraille with tactile graphics – replaced with Braille

  • Extended testing time over multiple days (online/paper) – replaced with Extended Testing Time Over Multiple Days

  • Human Reader for items – replaced with In-Person Human Reader

  • Human reader for repeat of response options one time – replaced with Repeat In-Person Human Reader

  • Human reader for repeat test items – replaced with Repeat In-Person Human Reader

  • Human reader for response options – replaced with In-Person Human Reader

  • Scribed Response – replaced with Scribe

  • Test Administered in a Non-School Setting - replaced with Test May Be Administered By School Personnel In Non-School Setting

  • Read Aloud (EAL Reading Passages) - Blind students... - replaced with Read aloud allowed for students at any grade level who do not yet possess adequate Braille skills. The use of this accommodation will not affect the student’s ELA score.

  • Special equipment - replaced with Specialized Equipment (see test manual) (NAEP)

  • Word Processor or Similar Keyboarding Device
