This article assists users when completing the Classroom Accommodations screen.
The screen is used to enter accommodations and modifications the student will use in the classroom. The selected accommodations print on Alternate Form F. Form F: Classroom Accommodations and Modifications. Accommodations will only print if Documented on Form F: Classroom Accommodations and Modifications is marked on the Services Screen.

This page includes instructions for entering program modifications/classroom accommodations. Remember to select “Documented on Alternate Form F” Form F: Classroom Accommodations and Modifications” in the Program Modification and Accommodations section on the Services Summary Form. This will create the Alternate Form F-Decision form: Classroom Accommodations and Modifications. Choose “Include Alternate Form F: Classroom Accommodations and Modifications in print layout” on the Decision Form. If this step is not completed Alternate , Form F: Classroom Accommodations and Modifications will not appear in the printed IEP, even if accommodations are entered on the Classroom Accommodations Screenscreen. If this happens and the IEP has been locked, an IEP Correction will have to must be completed.

Reminder The Classroom Accommodations Screen screen is not required by Phoenix. Therefore, it is possible for the IEP to be locked without entering any classroom accommodations. This could cause Alternate Form F: Classroom Accommodations and Modifications to be blank when printed, and no accommodations would be included in the IEP. If this happens and the IEP has been lockedstudent requires accommodations, an IEP Correction event must be completed to correct the error. Printing and proofreading the IEP before locking can prevent this. |
Adding Classroom Accommodations to the Plan
Open the Classroom Accommodations screen.
Select a Category to view a short list for
one category or All to view all accommodations.

The accommodations appear in Search Results. If "All" was selected the accommodations are grouped by category.
Select the classroom accommodations.
Add to Plan.

Completing Classroom Accommodation Details

Classroom Accommodations can be saved at any time. It is not necessary to complete all of the details before saving. If a field is blank, an error message will appear when the event validation is checked. The event cannot be locked if a required field is blank. |
Select the Accommodations with identical instructional areas by marking the appropriate checkboxes.
Click Select to choose the Instructional Areas (Location). The Instructional Areas window opens.

Select at least one area. More than one can be selected.
If "Other" is selected, enter the specific area in the Notes text box.
Select OK.

To edit the areas, select the link to reopen the window.
Click Select to
choose the Frequency. The Frequency window opens.

Select one frequency.
If "Other" is selected, enter the specific frequency in the Notes text box.
Select OK.

If necessary, edit the Begin Date or End Date.
Click the date link. The calendar opens.
Use the navigation tools to move to the correct month and year.
Select the date.
If "Other" was selected, enter the specific accommodation details in the text box.
When the details
have been entered, Save.

Deleting Accommodations Accommodations can be deleted by selecting the delete icon. The Confirmation window will openopens. Select Yes. The accommodation is deleted. |
