Form information

  1. Form B is created when Yes, the student is eligible for ESY services is

chosen
  1. selected on the Special Considerations form.

  2. There is only one option on Form B. Choose The Student is eligible for ESY Services.

  3. Save.

Info

Goals to be addressed during ESY are indicated entered then selected on the Goals screen.  The phrase “Will be addressed during ESY” will print prints on the goal form next to for each goal selected for ESY.

Services to be provided during ESY are entered on the Services screen. After opening the screen, select ESY Service as the Service Group Type before searching for the service to add to the plan. If the services are not entered when the event is locked, Form B will be blank when printed and a correction event or another amendment must be completed.
