Use this article to learn how to create a ROS ticket, tips on completing the form, reassigning the form and closing the form.

This is a very large form. Fill out 1 section at a time and click SAVE. SAVE often. Filling out the entire form over a long period of time vs your internet connection, may have problems. SAVE OFTEN. |
A) Creating a new ticket
Navigate to the SSD Portal, https://portal.ssdmo.org and login with your SSD username and password.
Click the Applications menu.
Click on the Eform Author/Restraint/Seclusion button.
Click the select link for Eform(s).
On the Main Menu screen, click on the Add Restraint link.
B) Completing a form

The ROS Form - Edit screen has a "DATA ENTRY ISSUES(S)" area at the top of the form. These are brief descriptions for sections of the form that must be addressed. This action must be blank in order to reassign, ESign, or close the form. |
DATA ENTRY ISSUE(s) section
SEC # refers to the section of the form
Description of the item you must take action on.

Complete one section at a time and click the SAVE link to preserve the form contents, then complete the next section. |
C) Reassigning the form
There are three items that are required to reassign a form.
DATA ENTRY ISSUE(s) must have all items addressed and must be blank.
Change the AssignTo drop-down from your name to your manager's name.
Check the the Author box in the E-Signatures section at the bottom of the form.

D) Closing the form
Closing a from requires three items.
DATA ENTRY ISSUE(s) must all be addressed and the section must be blank.
The Date Form sent to Parent/Guardian field must have a valid date entered.
The Manger box in the E-Signatures section at the bottom of the form must be checked.
Click the Save link to submit the form.

Once all of these items are completed the form status will update to CLOSED. |
